Enrollment Forms Mailed to Student Homes

Existing Students
MGS will mail out existing student enrollment forms for parents to complete at their leisure. Parents can bring completed enrollment forms to the Grade School during enrollment hours listed below. Text book fees of $47.00 will be collected during enrollment. Parents may also put money into their student's lunch account.
Breakfast $1.30 per meal
Hot lunch cost $2.10 per meal (increase of .10 cents per meal)
Milk (.35) per carton.
August 4th, 2008 11:00 am to 7:00 pm
August 5th, 2008 11:00 am to 7:00 pm
August 6th, 2008 8:00 am to 3:30 pm.
Please note: if your address has changed forms may not reach your new address.
New to the District / Returning to the District
Students new the district need to provide the following at the time of enrollment:
* Proof of Residency (i.e. parents drivers license, utility bill, lease/rental agreement or purchase agreement with current Mulvane address)
* Student's original birth certificate
* Student's social security number
