Enrollment Information and Curriculum Guide

For enrollment information call 777-1183. All forms must be filled out in person at Mulvane High School. You will need the following information:
- Proof of residency in the Mulvane School District: Examples included: driver's license, a bill with the current address, rental agreement, contract for purchase of home etc.
- Withdrawal paperwork from the previous school if transferring during the school year
- Copy of a last report card, transcript or progress report to begin work on a schedule
- Updated immunization records (if not included in the previous school's files)
- Birth certificate (if not included in the previous school's records)
